 Starting on the day that your treatment is presented, you will have sixty (60) days to earn Referral Rewards Discounts.
• For each new patient that you refer that comes in for an appointment in the next 60 days, you will receive 5% off of    your treatment.
• You are eligible to receive up to 50% off of your treatment for referring your friends and family to me.
• After 60 days you will continue to receive a $25 credit on your account to use towards future treatment for each new patient that you refer to my office.
